This paper tackles some of the paradoxes which arise from the application of the identity of indiscernibles to epistemic, modal, and definitional contexts, from a Thomistic point of view. The crucial notion is that of the intentional subject of a judgement, which is that by which the mind gains access to the referent of the subject expression. It is, as it were, the mode of presentation of the referent; but astonishingly, the authors do not refer to Frege's ``Über Sinn und Bedeutung''.
