The authors apply the general ideology of descent theory to provided a classifications of forms both in the algebraic and homotopical language using the theory of higher groupoids, similarly to the first author, \textit{D. Nikshych} and \textit{V. Ostrik} [Quantum Topol. 1, No. 3, 209--273 (2010; Zbl 1214.18007)]. In particular, the theory of categorification of weak unital-based rings by rigid tensor categories over arbitrary fields is discussed.
