The authors study the (non)-degeneracy of a certain class of quadratic forms giving a necessary and sufficient condition for the non-degeneracy of a given quadratic form. Using a defining set of the quadratic form they construct linear codes that are two-weight or three-weight codes and determine the weight enumerator of these codes. Certain related codes are shown to be optimal with respect to the Griesmer bound.
